Construction of domain knowledge graph of dementia care

Objective To construct a domain knowledge graph of dementia care,so as to provide the foundation and guarantee for the next intelligent application based on the knowledge graph. Methods A top-down approach was adopted to construct a domain knowledge graph of dementia care. Firstly,the ontology concept is constructed from the top level,namely the schema layer of knowledge graph. Then,instances are filled,and knowledge extraction is carried out from the existing data sources,and the extracted entities and relationships are filled into the pattern layer ontology database to complete the data layer construction of the knowledge graph. Finally,the “entity relationship entity” triplet data was input into the Neo4j graph database for storage. Results In this study,the personalized care plan set of 1 012 dementia cases was used as the corpus to construct a domain knowledge graph of dementia care. The knowledge graph takes people with dementia as the core,and unfolds,one by one,around basic characteristics,care problems,and care plans in a standardized “entity-relationship-entity” triplet format,forming a large knowledge network,which contains a total of 1 522 specific dementia care knowledge entities and 8 kinds of inter-entity relationships. Conclusion The domain knowledge graph of dementia care constructed in this study clearly and intuitively shows the global pedigree and logical path of knowledge,which provides an efficient and intelligent basic guarantee for the browsing,retrieval and application of dementia care knowledge,so as to realize personalized and intelligent management of people with dementia,break through the bottleneck of lack of professionals,improve the health outcomes of people with dementia,promote the implementation of inclusive pension services,and promote healthy aging.

This article summarized the nursing care experience of a pediatric patient with kawasaki disease after off-pump coronary artery bypass grafting. The key points of nursing care include:①the emergency treatment plan of chest pain was formulated before operation to prevent and identify myocardial infarction as soon as possible;early respiratory function training was carried out to improve cardiopulmonary function;risk management was improved to prevent rupture of coronary artery aneurysm. ②The circulatory system was closely monitored to ensure stable hemodynamic performance during operation;fluid balance was maintained to prevent renal function injury.③Posto-perative pain and irritation were managed to prevent atrial fibrillation;the coagulation function was monitored,and coagulation associated complications,such as epistaxis,should be treated timely;non-pharmacological intervention measures were taken to relieve postoperative delirium;individualized exercise rehabilitation care plan was made to enhance recovery;individualized diet plan was made to improve the nutritional status;structural education was enhanced to make the patient and his parents more cooperative. After careful treatment and nursing care,the patient recovered and was discharged from the hospital at 16 days after the surgery without any complications in one-year follow up.

Development and application of a follow-up checklist for postoperative patients with cervical cancer

Objective To formulate a scientific and standardized follow-up checklist for postoperative patients with cervical cancer,and provide a scientific tool to support the follow-up work. Methods The first draft of the follow-up checklist was constructed through literature review and semi-structured interviews,and the final draft of the follow-up checklist was formed by conducting 2 rounds of correspondence with experts using the Delphi method from October to December 2022,screening experts’ opinions and revising and improving the checklist. The convenience sampling method was adopted to select 8 nurses and 15 patients with cervical cancer after surgery from a tertiary A hospital in Liaoning Province from February to March 2023 as the research subjects. The nurses made telephone follow-up visits to the patients with the postoperative follow-up list and evaluated the follow-up list.Results The positive coefficients of the experts in the 2 rounds of correspondence were 88.89% and 100%,respectively;the authoritative coefficients were 0.88 and 0.89,respectively;the Kendal harmony coefficient of the primary,secondary and tertiary indicators were 0.177,0.245 and 0.179 in the first round and 0.170,0.118 and 0.107 in the second round,respectively(P<0.05). The final follow-up checklist for postoperative patients with cervical cancer included 4 primary indicators of physical and daily life support,information support,psychological support,and social support,12 secondary indicators,and 45 tertiary indicators. Both nurses and patients believed that the follow-up list was comprehensive with strong professional and clinical practicability. Conclusion The follow-up checklist is reasonable in structure,comprehensive in content,and reliable,specialized and practical,which can provide a scientific and standardized tool to support post-discharge follow-up of patients with cervical cancer.

To summarize the perioperative nursing care of a patient with laryngeal cleft complicated with laryngomalacia after endoscopic laryngeal cleft repaired. The key aspects concerning nursing included the impose of importance on nutrition management to improve preoperative nutritional status,the implementation of aspiration risk assessment and prevention strategies to reduce the risk of aspiration pneumonia before surgery, the strengthening of postoperative care to prevent anastomotic rupture,the strengthening of airway management to improve postoperative pulmonary complications,the development of a care plan to improve discharge readiness. Following appropriate and successful perioperative treatment and nursing care,the patient was recovered and discharged from hospital 28 days after operation,and showed good recovery after 1 year of following up.

Objective To evaluate the effectiveness of the shared medical appointments (SMA) in the management of type 2 diabetes mellitus (T2DM) and its impact on self-management behavior and efficacy and quality of life led by specialist nurses. Methods 30 patients with T2DM going to the endocrine clinic of a hospital from September to December 2020 were selected to participate in the SMA. The body mass index,Summary of Diabetes Self-care Activities (SDSCA) scale and Diabetes Management Self-efficacy Scale (DMSES) and Diabetes Specific Quality of Life (DSQL) scale were compared before the intervention and 3 months after the intervention. Results After 3 months of intervention,there was no statistical significance in the control rate of blood glucose compared with that before intervention (P=0.310). Waist circumference,total cholesterol and triglycerides of the patients after the intervention were significantly lower than those before the intervention (P<0.05). The total scores and the score of each dimension (diet,foot care) in SDSCA,and the DMSES score after the intervention were higher than those before (P<0.05). After the intervention,the total scores of DSQL and the scores of physiological dimension,psychological dimension and social relationship dimension were significantly lower than those before the intervention (P<0.05). Conclusion The SMA can effectively improve the waist circumference,total cholesterol and triglycerides of diabetic patients,improve the self-management ability,self-management efficiency,and the quality of life of patients with type 2 diabetes.

Investigation on the use and management of perioperative implants in 1225 hospitals in China

Objective To investigate the current situation of implant use and management in 1225 hospitals in China,and to analyze the existing problems,so as to provide references for the standard management of perioperative implants,and lay a research foundation for the formulation of relevant standards. Methods In May 2021,stratified sampling method was adopted to select operating room management personnel from 1225 hospitals in China. The persons in charge were set in the provinces,autonomous regions and municipalities directly under the central government. All the persons in charge were members of the surgical equipment and materials professional group of Nursing Equipment and Materials Branch of China Medical Equipment Association. Questionnaires were made through the Questionnaire Star and distributed and collected through the WeChat group. A self-designed questionnaire was used,including general information and “Questionnaire on Current Situation of Use and Management of Perioperative Implants”. Results In terms of implant management mode,there were mainly 3 modes,namely three-level management of the equipment department,operating room and specialist group(72.73%),on-board management of the equipment department(12.24%),and self-management of the clinical use department(10.69%). There were great differences among different types of hospitals. In terms of receiving foreign devices and implants,there are still some problems,such as late delivery of suppliers and non-standard charges. In terms of implant traceability management,68.90% introduced an information system to trace the use of implants;62.29% carried out information traceability of the cleaning,disinfection and sterilization process of foreign instruments and implants;48.89% carried out scanning charge and traceability management of implants,indicating that information construction needs to be strengthened. In addition,there are many problems and safety risks in the treatment of unused implants after removal or unpacking and related training,which need to be improved. Conclusion Relevant departments and managers should improve the relevant system and process management according to the scale and actual conditions of different hospitals,strengthen the supervision of implant access,and the whole process information tracing management. Relevant standards should be established in time for non-use of implants after removal and unpacking,so as to fill the management gaps. Supervision over the implementation of existing standards should be strengthened.

Design and application of hypoglycemia nursing decision support system

Objective To construct a hypoglycemia nursing decision support system and to explore its application in clinical practice. Methods This study firstly established a nursing decision support system of hypoglycemia,and realized functions such as intelligent entry,intelligent reminding,and assistant decision making. The medical records before the application of the system(from January to March 2018) and after it(from October to December 2018) were selected to compare the incidences of hypoglycemia and the completion of nursing documents. 117 nurses were enrolled to investigate their satisfaction with the system by self-designed satisfaction questionnaires. Results After applying the hypoglycemia nursing decision support system,the recurrence rate of hypoglycemia decreased from 7.05% to 3.33%,and the difference was statistically significant(χ 2=8.253,P=0.004). The completion rate of nurses’ documents increased from 68.17% to 94.23%,and the difference was statistically significant (χ 2=137.303, P<0.001). 117 questionnaires were collected,and the average score for each dimension was over 2.50. The results showed that nurses were more satisfied with the overall system. Conclusion The establishment of a hypoglycemia nursing decision support system can reduce the recurrence rate of hypoglycemia,improve the quality of clinical nursing documents and the efficiency of clinical nurses,and increase the satisfaction of nurses.

Analysis of health status and its influencing factors of community residents based on the intelligent health monitoring system

Objective To investigate the health status of community residents and to analyze related influencing factors. Methods A descriptive cross-sectional study was conducted to obtain data of 4 311 community residents in China from April to May,2019. Intelligent health monitoring system,including their general characteristics,health behavior,health status,results of generalized anxiety disorder scale-7(GAD-7) and patient health questionnair-9(PHQ-9),was used as the suvery tool. Descriptive statistics,chi square test and logistic regression were used for data analysis. Results The average age of the participants was(29.25±11.88) years old,and the prevalence of chronic diseases was 13.8%. There were 1 480(34.3%) patients with anxiety and 1 620(37.6%) with depression. The smoking rate and drinking rate were 12.2% and 32.3% respectively. Age,education level,being married,meal regularity,additional meal in the morning,eating dinner,meat-based diet,salty diet,other special diet,drinking alcohol,taking vigorous exercise,anxiety and depression were the influencing factors of chronic diseases. Conclusion The health condition of community residents was generally good,while the symptoms of anxiety and depression were common,which implies that much attention should be paid on the emotional health. The risk factors of chronic diseases were old age,being married,irregular meal,meat-based diet,salty diet,other special diet,drinking alcohol,no vigorious exercise,depression and anxiety. These findings can provide suggestions for the community to develop interventions to promote residents’ health.

Design and application of informationalized nursing dashboard in general wards

Objective To design and develop a two-dimensional informationalized nursing dashboard for clinical nurses’ decision-making in general wards,so as to solve the manual identification of patients’ important information and cross-space communication,providing nurses with accurate and intuitive information display and communication channels.Methods The information items of the two-dimensional informationalized nursing dashboard were confirmed by current situation investigation,literature search and expert meeting. Data were automatically acquired from the hospital information platform,and then were analyzed,classified and presented. After used by nurses in general wards in our hospital for half a year,its effects were evaluated using interview method.Results The nursing informationalized dashboard extracted key information from two dimensions(whole wards and single patient),including 62 items in four categories,namely 15 items of patient data,37 items of nursing evaluation,9 items of nursing tasks,and 1 item of remark information. It realized the direct reading and real-time updating of data,improved working efficiency of nurses and brought positive psychological feelings to nurses.Conclusion The two-dimensional informationalized nursing dashboard can realize automatic information transmission,resource sharing and change the inherent clinical nursing working mode,which is practical and innovative.
